The File of Life program was officially rolled out at the April 28 2007 Are You Ready? Fair.  Over 2000 Kits have been given away at 20 sites including Medical Offices, Council on Aging, EMS , Health Department, community college, Churches, and places of employment.  The Program was underwritten by Safe Communities, and the Kits cost the Council one dollar a piece. 

 

Each kit contains a medical information form, a plastic magnetic sleeve for the fridge, and a sticker for the front door to alert emergency medical services that the resident has the file of life form inside.  The purpose of these kits is to have critical medical information on-hand.  In the event of an emergency, a patient may be unable to communicate key information such as allergies or who their preferred medical provider is.  The File of Life can be taken with the patient or provider to the hospital emergency room.  File of Life would also be useful to take to emergency shelters during disaster response.

 

This downloadable copy comes without the red plastic sleeve, the front door sticker, and the magnet.  However, the information entered on this form could save your life.  So please download it, enter your information, and we hope you never have to use it. 

2000 Wallet sized File of Life forms and sleeves were also distributed for placement in automobiles and wallets.
